Criminal Record Check
Criminal Record Checks are required for various reasons and purposes:

Our service provider for criminal record checks is a certified and accredited civil fingerprint agency.
There are two options for completing your criminal record check:

Online Criminal Record Check
If you DO NOT have a criminal record, you only need to complete the “Informed Consent Form” (page one).
If you know that you have a criminal record, you must also complete the “Declaration of Criminal Record Form” (page two). If you do not have all the information relative to the Offence, Date of Sentence and Court Location, we can find this information using fingerprint analysis, as we are a direct-connect to the RCMP database. Please complete as accurately as possible, with as much information known.
To verify your identity, you will need to provide CLEAR (non-blurred, non-glare) copies of TWO pieces of Government Issued Identification, ONE of which MUST BE PHOTO ID. These are to be uploaded with your Informed Consent Form. Acceptable Identification include:
- Driver’s License or Identification Card
- CNIB ID Card
- Birth Certificate
- Indian or Metis Status Card
- Citizenship Card
- Permanent Residency Card
- Firearms Acquisition Card
- National Defense Card
- Win, AISH or Canadian Blood Service Card
- Provincial Health Care Card (full name—not initials)
- Social Insurance Number Card
If you have difficulty uploading your identification, you will be required to bring a CLEAR copy to the IRISS Corporation offices.
Instructions for Signing Documents and Uploading ID
If you are using the online Criminal Record Check, designated signature spaces allow you to use your mouse, stylus or finger (depending on the device you are using) to sign the document. A signature is required to process your request.
You can upload or drag and drop CLEAR copies of your identification at the bottom of the form. The ID document section will only appear after you answer the question “Do you have a criminal record?”. If you answer “no” to this question, you will be presented with the identification section right away. If you answer “yes”, Page 2 questions (Declaration of Criminal Record) will appear, followed by the identification section.
Once you have completed your online form, and attached your identification, please click the SUBMIT button. Your next step is to submit payment. See instructions below.
Download and Email Criminal Record Check Form
If you are unable to complete the online process, download and complete the forms according to the instructions above, and email them with CLEAR (non-blurred, non-glare) copies of TWO pieces of Government Issued Identification, ONE of which MUST BE PHOTO ID (see acceptable identification above) to firstname.lastname@example.org
IF YOU ARE UNABLE TO SUBMIT YOUR REQUEST FOR YOUR CRIMINAL RECORD CHECK THROUGH EITHER OPTION NOTED ABOVE, PLEASE EMAIL ADMINISTRATION AT email@example.com AND ARRANGEMENTS WILL BE MADE FOR YOU TO ATTEND THE IRISS OFFICE.
In order to process your criminal record check, payment of $65 (CAD) must be submitted immediately upon submitting your form. This site does not currently accept payment for this service. Payment is to be made via e-transfer and should be sent to firstname.lastname@example.org immediately upon submitting your request. YOU MUST INCLUDE the following message in the comment line of your e-transfer “Background Check – (Your First Name and Last Name)” so your payment can be cross-referenced with your record check request.
A successful submission will produce results of your criminal record check within 48 hrs. Results will be emailed to the email address provided on the “Informed Consent Form” that you have submitted. If you have not received results within 3 days upon submission, please follow up via email email@example.com
Failing to complete and provide the required documentation as specified may cause delays or disallow your request for a criminal record check to be processed.
